Transition of Care
We know change can be scary. Whether you are new to Iowa Total Care or changing health plans, we are here for you. We want to make this change easy. We will make sure you continue to get the services you need.
One big challenge when changing Medicaid plans is staying with the providers you know well. Our staff will work with you and your providers. We will help you continue any services you are receiving even if the provider is not in our network.
Let us know right away if you are getting services that you need to continue. We can provide you with the information you need to continue your services. The information will guide you on how to switch health plans and continue to receive the services you need.
We will honor authorized services for 30 days. After that time, we may require authorization for the service. We will let you stay with an out-of-network provider for up to 30 days. In some cases, you may be able to keep your out-of-network provider for a longer period.
We will help you find a provider in our network that can provide the services you need. Our case managers can help get your medical records transferred to your new provider.
