We are excited that you selected our provider network as your network of choice.
As an Iowa Total Care provider, you will have access to:
- A comprehensive approach to care for your patients through disease management programs and healthy
behavior incentives. - Initial and ongoing provider education through orientations, office visits, training and updates.
- A dedicated claims team to ensure prompt payment.
- Minimal referral requirements and limited prior authorizations.
- A dedicated provider relations team to keep you informed and maintain support in person, by email or by phone.
- An online Secure Provider Web Portal where you can check member eligibility, submit and verify prior authorizations and verify claims status.
- Healthcare collateral for your patients (e.g., information about our benefits and services) and educational displays for your office.

Expanded, cost-effective mental health and substance use services offered by contracted providers to Medicaid members through the 1915(b) waiver.

Home- and Community-Based Services (HCBS) Waivers:
Provides financial assistance and personalized support for eligible individuals.
- Supports independence
- Allows individuals with functional, cognitive and other physical/mental health needs to receive care at home or in the community.
- Expanded benefits
- Because these are funded through Medicaid’s HCBS waiver programs, members enrolled in waivers can access Medicaid covered services.
Habilitation Services:
Provides personalized support for eligible individuals with serious mental illness (SMI) or serious emotional
disorder (SED).
- Supports independence
- Assist members in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in the home and community.
- Expanded benefits
- Members of any age qualify if their income is at or below 150% of the federal poverty level and they meet at least one of five risk factors and two of five assistance needs.
How to Enroll for Waivers and Habilitation Services:
- Visit the Iowa Department of Health and Human Services' Provider Enrollment webpage to apply to become a Medicaid HCBS provider.
- Once you receive your approval letter, submit a copy to Iowa Total Care’s Network Management team: NetworkManagement@IowaTotalCare.com
