Attention Pharmacy Prescribers: New Fax # on Prior Authorization Forms on ITC Website

This alert is to remind prescribers that today, July 1, 2021, Iowa Total Care (ITC) pharmacy claims will now be processed through CVS Caremark. As part of the migration to CVS, the fax number on the Prior Authorization (PA) forms has changed. All forms on the website include the new number.
