Home- and Community-Based Services (HCBS): Provider Documentation Needed During Member Visits

Effective February 1, 2026, Iowa Total Care will gather and review HCBS provider documentation for all members at least quarterly. Iowa Total Care community-based case managers will review provider documentation as part of
on-going service monitoring and follow-up.

Providers should have documentation available for the community-based case manager to review during member visits. It includes reviewing the provision of services to ensure the member is receiving the services in the amount, duration and scope identified in the comprehensive person-centered service plan. 

Reviews may include but are not limited to the following: 

  • Service plans
  • Service logs
  • Notes or narratives
  • Mileage and transportation logs
  • Financial records
  • Invoices or receipts 
  • Medications
  • Medication Administration Record (MAR)
  • Incident reports
  • Other service documentation (as applicable)
