PROVIDER NEWS ALERT: CONSUMER-DIRECTED ATTENDANT CARE (CDAC) PAPER CLAIMS AND ADDRESS CHANGE/INDIVIDUAL PROVIDERS

Paper Claim Submission

Paper claims may be submitted to Iowa Total Care using the Consumer-Directed Attendant Care (CDAC) Targeted Medical Claim Form.  Click here for the CDAC Targeted Medical Claim Form. 

CDAC individual claim forms MUST be mailed to the following address:

Iowa Total Care does not accept faxed CDAC claim forms.  Faxed CDAC claim forms will be rejected and returned to the Provider to mail to the Iowa Total Care address noted above.  Faxing claim forms will delay payment.

Address Change

Iowa Total Care requires an updated W-9 Tax ID Form if there is an address change.   Email the updated W-9 Tax ID Form to NetworkManagement@IowaTotalCare.com.  Click here for the W-9 Tax ID form.
