Oxygen Concentrator (Oxygen in the Home)

Effective November 1, 2022, pre-authorization is not required for procedure code E1390, Oxygen Concentrator for participating (PAR) providers with home location (12).
Participating (PAR) providers will need an authorization in any location other than home (location 12).
Non-participating (Non-PAR) providers will need an authorization in all instances. Please reference chart below for a quick reference guide of when an authorization is needed.
